Not a book, but Mark and Mike at Manager Tools have done a number of podcasts on meetings: staff meetings, teleconferencing, skip levels, large meetings, brainstorming, and more. I'd suggest starting with the first part of their Effective Meetings cast.

I think they provide some very good, practical advice, and you'll also find some other related resources there, such as their agenda template and a PDF outline of their "formula" for effective meetings. Check it out.
